second tab come from parameter "Frontend Language" in Administration->Users->Manage->Option
third tab come probably from some other user plugin, but I don't know, I need check your configuration from your backend, can you provide me your site details?

Easy Profile extends Joomla user management (not replace), so al plugin that works with Joomla will works also with Easy Profile (some user plugin can have some problems with tabs because it not have a title in Joomla registration form).

you have a modded version of plugin called "User - Profile", this is a Joomla built-in plugin disabled by default in all Joomla installations.

I'm sorry but is not possible to disable middlename only in registration form. You can simply hide with CSS, try with this CSS code:

#member-registration .secondname-group{display:none;}
