I am unable to view the form properly as the fields are the same colour as the background. Is it that the CSS for the form is not loading?

In your template missing some style for HTML input type, for example, you can try to see your login page at http://www.tvjsn.com/demo/login.html, this page is not by Easy Profile but you have same issue.
