sorry for late reply.
Installation Problem:
from version 1.3.0 we have added new column in ezrv_jsn_fields table.
If you update from 1.2.x to 1.3.x so no problem the system update your table, but if you have installed 1.2.x and removed, Joomla make installation as new and not as update.
The solution is this: install old version and update with new. I know you have re-installed Joomla, but I write a solution for other customers
